Former Roswell Infusions patients may initiate a request for copies of their medical records by emailing: roswellinfusions@gmail.com

Do not include medical information, diagnoses, treatment details, or other protected health information in the initial email.
After your message is received, instructions will be provided for submitting the request through a secure method.
Medical record requests may be subject to a reasonable retrieval or administrative fee when permitted by applicable law and required by the former electronic health record provider or records custodian.
